Please Note: All members of your practice group must submit dues in order to receive the discounted price. You must also use the AOS Group Annual Membership Template (Excel spreadsheet found below; works best in Safari or Chrome) when sending the list of physicians. After filling out the spreadsheet it must be saved in one of the following formats: .XLS or .XLSX.

Cost: Pay by Check - $400 per person | Pay by Card - $414 per person (includes a 3.5% administrative charge for all credit cards)

Active Members Only Please Note: Before registering, please check with your practice administrator to determine whether your group qualifies for discounted group membership pricing.
